I was running product for a team of seven engineers and a designer when we shipped a release that broke the invoice flow for our second-largest customer. Not a subtle break — invoices rendered with the decimal point shifted two places, which meant $12,400 showed as $1,240,000 on the PDF the customer’s CFO pulled for a board meeting. The bug was in a module we’d touched twice in six months. QA hadn’t regression-tested it because QA didn’t know it was in scope. The engineer who changed it hadn’t flagged it because he’d unit-tested his change and the unit test passed. The designer hadn’t caught it because designers don’t look at PDFs. And I — the PO — hadn’t caught it because I’d reviewed the acceptance criteria, confirmed the logic change was scoped, and moved on to the next story. The release went out on a Thursday. The customer called on a Friday. The CFO presentation was Monday.

What stung wasn’t the bug. Bugs happen, and we fixed it in four hours. What stung was the realization that I’d been running a team of seven for three months and had never once asked the question: who owns the guarantee that this thing works end-to-end? I’d been writing stories, prioritizing the backlog, running standup, doing stakeholder updates, reviewing designs, and answering support escalations. I was, by any visible measure, busy. But I’d left a hat on the table — the QA-ownership hat — and nobody else had picked it up, because at seven people, nobody else’s job description covered it either. The hat was mine. I just didn’t know it was on the floor.

This is the pattern I want to trace through this post: the PO role doesn’t change once as teams grow, it changes shape continuously, and there’s a specific team size — roughly eight people — where a hat drops and nobody notices until something breaks. I’ll map the hats by team size, explain which ones the PO keeps and which ones get delegated, and focus on the one that almost always gets orphaned.

The hats a PO actually wears

When I say “hat” I mean a responsibility that someone must hold — not a task, but an ongoing ownership that doesn’t complete when the task does. A task finishes; a hat stays on. The PO role, as I’ve lived it across teams of three, five, eight, twelve, and twenty, carries roughly a dozen hats. Not all at once — which ones stay on the PO and which ones migrate off is a function of team size, and the migration pattern is predictable enough to be useful.

The hats, stated as owner-not-delegated until the migration:

Discovery and opportunity ownership. Understanding customer problems, running interviews, maintaining the opportunity space. This is the hat that defines the role in the continuous discovery series, and it’s the first hat most POs are hired to wear. It stays on the PO at every team size, and when it migrates, it migrates to a dedicated researcher or a product trio — it doesn’t disappear.

Backlog authorship and priority. Writing user stories, ordering the backlog, making tradeoff calls when capacity doesn’t fit desire. This hat lives with the PO from the first sprint. At larger teams it gets thinner — the PO writes fewer stories and more acceptance criteria, and prioritization becomes a cadence artifact rather than a weekly negotiation — but it never fully leaves.

Stakeholder communication. The pre-wiring — translating between the team’s work and the organization’s needs. At small teams, this is the PO’s entire external-facing job. At larger teams, it expands into what I’ve described elsewhere as the intake system — the structured way asks enter the roadmap. The hat stays on the PO, but the mechanism changes.

Sprint facilitation. Running standup, unblocking the team, managing the sprint board. This is the hat that migrates earliest and most visibly. By eight engineers, the tech lead has usually taken standup, and that’s right — the PO facilitating a standup of eight engineers will either make it too long or too shallow, and the engineers need the space to coordinate among themselves. The migration is healthy. The problem is what happens next.

Design review and UX ownership. Ensuring the product makes sense to the user, not just the buyer. At small teams the PO and designer share this hat — the designer owns the artifact, the PO owns the user context. As teams grow, the designer becomes more autonomous, and the hat migrates to them fully. Again, healthy. The PO stays close but stops being the design’s second-guesser.

Technical feasibility assessment. Understanding enough about the architecture to know what’s cheap and what’s expensive. At small teams the PO wears this hat alongside the tech lead. At larger teams, the tech lead owns it fully. The PO’s version of feasibility is usually softer — “is this a two-week or a two-month bet?” — and the tech lead’s version is sharper — “this touches three services and the migration path is non-trivial.” The migration is gradual and usually complete by twelve engineers.

Release management. Knowing what ships when, coordinating the deployment, managing the feature flags. At small teams the PO often wears this hat because someone has to, and the PO is the person tracking what’s done. As teams grow, a dedicated release manager or the tech lead absorbs it. The migration is one of the clearest signals that the team has outgrown the three-to-five range.

Customer support triage. When a ticket comes in, deciding whether it’s a bug, a feature request, or user error — and knowing which of those it is fast enough that the support team trusts the product team. This hat sits with the PO at every team size I’ve worked with, though at larger teams the PO’s version becomes “pattern recognition across tickets” rather than “ticket-by-ticket triage.” The signal pipeline concept is the systematic version of this hat.

QA ownership. This is the hat nobody writes. Not in the job description, not in the sprint ceremony, not in the definition of done. And at roughly eight people, it falls through the gap.

The orphan gap at eight

Here’s why eight is the breaking point, and I want to be specific about the mechanics, because it isn’t a vague “the team got too big” story. It’s a structural failure with a clear timestamp.

At three to five engineers, QA ownership lives implicitly in the team’s collective muscle memory. Everyone tests everything. The PO reviews acceptance criteria and often manually tests edge cases before release. The engineer who writes a change is the same person who tests it in adjacent scenarios. The designer catches UI regressions because the team is small enough that the designer touches every screen. There’s no formal QA hat because the team’s surface area is small enough that collective ownership actually works. This is the sweet spot where most three-to-five-person teams ship, and it’s genuinely fine.

At five to seven engineers, the cracks start to show. The team has too many active stories for any one person to test all of them. The PO stops manually testing because stakeholder communication and discovery consume the hours that testing used to occupy. Engineers still test their own changes, but the cross-module regression — the kind where a change in billing breaks an edge case in reporting — falls through the gap between individual ownership and collective ownership. The PO notices this and usually responds by tightening the acceptance criteria, which helps but doesn’t solve the underlying problem: nobody is the person whose job it is to verify that the whole system works, end-to-end, before a release goes out.

At eight engineers, the hat is officially orphaned. The team has enough parallel work that individual ownership of cross-module impact is impossible. The PO has enough non-technical hats that manual testing is a ritual, not a discipline. The tech lead is managing architecture, not regression coverage. And the organization hasn’t hired a QA engineer because the team was “doing fine without one” at five people, and nobody made the explicit call to add the hat. So the hat sits on the floor.

The invoice bug I described at the top of this post happened at exactly this stage. Eight engineers, a designer, me as PO. We had a release process that was: engineer marks story done, PO reviews acceptance criteria, tech lead approves the PR, we deploy. What we didn’t have was anyone checking that the stories, in aggregate, produced a working product. The acceptance criteria for each story were met. The system-level behavior was broken. The gap between story-level acceptance and system-level quality is exactly the gap QA ownership covers, and at eight people, we had no one covering it.

The matrix: which hats migrate, and when

Here’s the pattern I’ve observed across teams, stated as the team size at which each hat typically migrates off the PO — and the critical thing to notice is that the migration isn’t always to a named role. Sometimes the hat migrates to nobody, which is the orphan gap.

The QA row is the one to stare at. Every other hat migrates to a named person or a named team. QA ownership migrates to the floor. The reason isn’t that people don’t care about quality — they do. It’s that QA ownership is the one hat that doesn’t have a natural home after it leaves the PO. Sprint facilitation naturally lands with the tech lead because the tech lead is already coordinating engineering. Release management lands there too for the same reason. Design review lands with the designer because the designer owns the design artifacts. But QA ownership — the holistic, cross-module, end-to-end verification that the product works — doesn’t map to any individual contributor’s job description at a team of eight. The engineer owns their stories. The tech lead owns architecture. The PO owns outcomes. Nobody owns “the product as a whole works.”

How to fill the gap without hiring a QA engineer

I want to be practical here, because the answer isn’t always “hire a QA engineer.” At eight people, you may not have the headcount budget, or the organization may not believe in dedicated QA, or the team’s testing needs may be better served by engineering practices than by a manual-testing role. The point is that someone must own the hat, and the options range from hiring to process to culture.

Option one: the tech lead wears it. This is the fastest fix and the one I’ve used most. The tech lead adds “pre-release regression check” to their sprint responsibilities — a thirty-minute pass across the stories about to ship, focused on cross-module impact, not individual story acceptance. The tech lead is the person who understands the architecture well enough to know which changes are likely to ripple, and they’re the person the team trusts to make the call. The cost is thirty minutes per release. The benefit is that the hat has an owner. The risk is that the tech lead is already overloaded, and adding QA ownership to an already-full plate produces a hat that’s nominally owned but practically neglected.

Option two: rotate the hat. Each sprint, a different engineer is the QA owner for that sprint. Their stories are still their stories, but they also own the release-verification pass. Rotation distributes the knowledge — everyone learns the cross-module testing discipline — and prevents any single person from being bottlenecked. The risk is that the rotation produces variable quality, because not every engineer is equally skilled at finding cross-module bugs. The mitigation is a lightweight checklist: for each release, verify that every changed module has been smoke-tested in the integration environment, that no existing workflow in the affected domain has regressed, and that the deployment pipeline’s automated tests cover the happy path for each changed area.

Option three: hire a QA engineer. The cleanest answer if the budget allows, and the one that scales past eight people. A dedicated QA engineer owns the hat fully — they write test plans, they regression-test before release, they build automated test suites for the cross-module scenarios. The integration cost is real: the QA engineer needs to learn the system’s failure modes, which takes three to six months. The benefit is that the hat is permanently off the floor and the team stops carrying the implicit risk of untested cross-module impact.

Option four: shift-left with automated testing. Invest in end-to-end automated tests that run on every PR or on every merge to main. This is the engineering-infra answer: the hat is worn by the CI pipeline. The benefit is that it scales without headcount. The risk is that automated end-to-end tests are expensive to write, expensive to maintain, and prone to false confidence — a green test suite doesn’t guarantee the product works, it guarantees the test suite’s scenarios passed. The sweet spot I’ve found is automated tests for the critical paths (the workflows that, if broken, produce customer-facing bugs like the invoice decimal issue), combined with a human check for everything else.

The sign you’ve hit the orphan gap

There’s a diagnostic I use when I join a team of eight to twelve engineers: ask “who is the person who would notice if a release broke a workflow that no single story touched?” If the answer is “nobody” or “I guess the PO?” — the hat is on the floor. The PO noticing it is the illusion of ownership; the PO noticing it means the PO is doing the job in addition to their own, and the QA work is the first thing to get dropped when stakeholder pressure or discovery work fills the calendar.

A more concrete sign: the team has shipped three releases in the last quarter that each required a hotfix within forty-eight hours, and the hotfixes were for regressions — things that used to work and stopped working because of an unrelated change. Three hotfixes per quarter is the orphan gap’s fingerprint. It means the team’s release verification isn’t catching cross-module impact, which means nobody is verifying cross-module impact, which means the hat has no owner.

The invoice bug I described at the top was our second hotfix that quarter. The first was a navigation issue caused by a permissions refactor — a change that touched authentication and, unexpectedly, the menu rendering. No single story touched both. No single engineer understood both. The QA pass that would have caught it — someone walking through the user-facing workflows after the permissions change deployed — didn’t exist because nobody owned it.

What the PO keeps

By way of closing, here’s what the PO role looks like at eight engineers after the migrations I’ve described:

The PO keeps discovery and opportunity ownership — the continuous discovery habit stays non-negotiable. The PO keeps stakeholder communication and the intake system. The PO keeps backlog authorship, though it’s thinner now — more acceptance criteria, fewer story-level details. The PO keeps customer support triage at the pattern level — the signal pipeline is the systematic version.

The PO loses sprint facilitation to the tech lead. The PO loses release management. The PO loses design review to the designer’s growing autonomy. The PO loses technical feasibility to the tech lead’s deeper architectural knowledge.

And the PO should have lost QA ownership — but only if someone picks it up. If nobody does, the PO is still wearing it, invisibly, which means the PO is doing two jobs and the second one is the job nobody notices until it fails.

The invoice PDF was fixed in four hours. The CFO presentation happened Monday anyway — the customer printed a manual correction. We kept the account, barely. What I took from it wasn’t a better release checklist. It was the discipline to look at the team at eight people, name every hat that hadn’t been explicitly assigned, and ask the question I should have asked at seven: who owns the guarantee? If the answer is “nobody,” that’s not a gap in the process. It’s a gap in the org, and it’s the PO’s job to name it before the next release proves it.
